QIP-NJ Learning Collaboratives

Learning Collaborative (LC) Overview

To support hospitals in their effort to improve maternal health and behavioral health, DOH has run a succession of data-driven Learning Collaboratives. Each Collaborative has taught evidence-based best practices, provided a forum for peer-to-peer learning, and offered improvement coaching for frontline care teams. Participating in these Collaboratives has accelerated work already underway and helped teams tackle new challenges to achieve meaningful progress over time.

The QIP-NJ Behavioral Health Learning Collaborative (BHLC) focused on improving follow-up visits after an Emergency Department (ED) discharge. The Maternal Learning Collaborative (MLC) focused on inpatient maternity units and EDs as priority settings to test changes for improvements in maternal care processes and outcomes for the New Jersey's Medicaid maternity population.

Social Determinants of Health Learning Collaborative Program Information

The QIP-NJ Social Determinants of Health (SDOH) Learning Collaborative (SDOHLC) started in September 2024 and will end in May 2025.


Maternal Learning Collaborative Program Information

The QIP-NJ Maternal Learning Collaborative (MLC) launched in October 2022 and closed in December 2023. The following materials outline the program design, improvement strategy and expectations:
